The spin-lattice relaxation times (T₁) of water doped with various concentrations of paramagnetic species were determined using the inversion-recovery method. Intensity was measured directly from images as a function of tau and T₁ obtained from a three parameter exponential fit of the data. The effects of varying imaging conditions on the values of T₁ obtained, were also examined. The results were compared with T₁ values obtained using a two-point computational method which is available on many commercial imaging instruments. This involves taking the ratio of an inversion-recovery image and a spin-echo image, which eliminates the dependence of the images on the equilibrium magnetization and the repeat time. The spin-spin relaxation times (T₂) were determined using the spin-echo method, of water doped with the same concentrations of paramagnetic species used to study T₁. Intensities were again obtained directly from images as a function of 2 tau and T₂ obtained from a two parameter exponential fit of the data. The effects of diffusion on the values of T₂ obtained were also examined. The values were compared with those obtained from a two-point computational method, which takes the ratio of two spin-echo images with different tau-times. The T₁ and T₂ data were also compared with literature values obtained under conventional spectroscopic conditions, with no magnetic field gradients present. The results of these studies, which compare favourably with those in the literature, have shown that it is possible to obtain reproducible values of T₁ in the range 100-600 ms, with acceptable errors (±12%) under variable imaging conditions. Reproducible values of T₂ can be obtained in the range 40-200 ms, which have errors of ±15% or less. Above this range the effects of diffusion become important. Experimental allergic encephalomyelitis (EAE), an animal model for multiple sclerosis, was induced in a Macaca fascicularis monkey, and the development of the disease was followed using quantitative NMR imaging. This technique has been shown to be a powerful tool in the study of EAE in primates, since the progression of the disease is accompanied by changes in T₁ and T₂. The s l i c e t h i c k n e s s i s determined by the magnitude o f the magnetic f i e l d g r a d i e n t used, and the frequency bandwidth o f the s e l e c t i v e RF p u l s e . In December 1982, the prototype whole body NMR imaging system o f P i c k e r I n t e r n a t i o n a l was i n s t a l l e d i n the Extended Care U n i t o f the Health S c i e n c e s Centre H o s p i t a l on the U n i v e r s i t y o f B r i t i s h Columbia (UBC) campus. T h i s system i s p a r t o f the Imaging Research Centre, which a l s o i n c l u d e s a CT X-ray scanner and P o s i t r o n Emission Tomograph. The NMR imaging system c o n s i s t s o f an Oxford Instruments superconducting magnet, with a room temperature bore o f 1 metre, o p e r a t i n g a t a s t a t i c magnetic f i e l d s t r e n g t h o f 0.15 T e s l a (proton resonance frequency, 6.4 MHz). The system i s 9 i n t e r f a c e d w i t h a P e r k i n E l m e r c o m p u t e r , a n d a l 1 o p e r a t i o n a l s o f t w a r e i s w r i t t e n b y P i c k e r I n t e r n a t i o n a l ( s e e A p p e n d i x I f o r m o r e d e t a i l s ) . I f t h e s e i n h o m o g e n e i t i e s a r e l a r g e , t h e y w o u l d g i v e r i s e t o a r a n g e o f r e l a x a t i o n t i m e s , s i n c e t h e y a r e f i e l d d e p e n d e n t . 2. M a g n e t i c f i e l d g r a d i e n t n o n - l i n e a r i t y . T h i s c a u s e s d i s t o r t i o n i n t h e i m a g e . 18 3. S l i c e s e l e c t i o n . T h i s i n t r o d u c e s the problem o f volume a v e r a g i n g which i s d i s c u s s e d in d e t a i l in Chapter 2. 4. S p a t i a l r e s o l u t i o n . T h i s w i l l de termine the s m a l l e s t a r e a which can be s t u d i e d . 5. R e p r o d u c i b i l i t y . I f q u a n t i t a t i v e measurements a r e t o be o f d i a g n o s t i c use t h e y must be r e p r o d u c i b l e over t i m e . 6. Methods o f R e c o n s t r u c t i o n . These can a f f e c t t h e appearance o f the image and i n t r o d u c e a m b i g u i t i e s in the observed i n t e n s i t i e s . 7. So f tware L i m i t a t i o n s . C e r t a i n parameters (such as TR) have l i m i t s , which can a f f e c t the outcome o f a c c u r a t e q u a n t i t a t i v e measurements. At the o u t s e t , t h e s t r a t e g y o f t h i s work was t o c a r r y out q u a n t i t a t i v e measurements on s imp le sys tems , then p r o g r e s s t o more complex ones . T h i s proved t o be an u n e x p e c t e d l y d i f f i c u l t t a s k . I n i t i a l l y , much t ime was expended t o o b t a i n s p a c i a l l y encoded x s p e c t r a ' u s i n g F o u r i e r t r a n s f o r m a t i o n o f s i n g l e p r o j e c t i o n s from back p r o j e c t i o n data s e t s ( F i g u r e 10) o f v i a l s c o n t a i n i n g water doped w i th v a r i o u s c o n c e n t r a t i o n s o f c o p p e r ( I I ) s u l p h a t e (CuSO^). The i n t e n t i o n was t h a t the s i g n a l i n t e n s i t i e s f rom t h e s e * s p e c t r a ' c o u l d be used f o r c a l c u l a t i o n o f T J t w i thout a r t e f a c t s from any subsequent imaging o p e r a t i o n . I n a d d i t i o n , when s e t s o f d i f f e r e n t s o l u t i o n s w e r e s t u d i e d s i m u l t a n e o u s l y , f l u c t u a t i o n s i n s i g n a l s i g n s f o r i n d i v i d u a l s o l u t i o n s w e r e o b s e r v e d a n d p h a s e e r r o r s w e r e a p p a r e n t . One o f t h e m a j o r p r o b l e m s i n m a p p i n g t h e c o u r s e o f MS I s t h a t t h e o n l y d a t a a v a i l a b l e o n humans I s p o s t m o r t e m , b y w h i c h t i m e t h e d i s e a s e I s u s u a l l y i n t h e c h r o n i c s t a g e . T h i s means t h a t l i t t l e i s known a b o u t t h e p a t h o l o g y o f t h e e a r l y s t a g e s o f t h e d i s e a s e . F o r t h i s r e a s o n , a n i m a l m o d e l s f o r MS h a v e b e e n d e v e l o p e d i n o r d e r t o s t u d y t h e m e c h a n i s m o f t h e d e v e l o p m e n t o f t h e e a r l y l e s i o n . One s u c h m o d e l i s E x p e r i m e n t a l A l l e r g i c E n c e p h a l o m y e l i t i s ( E A E ) . NMR i m a g i n g h a s p r o v i d e d a n o p p o r t u n i t y f o r s e r i a l l y s t u d y i n g b o t h MS a n d EAE w i t h n o known r i s k t o t h e s u b j e c t . The a b i l i t y t o d e t e c t a b n o r m a l a r e a s i s a r e s u l t o f c h a n g e s i n t h e NMR c h a r a c t e r i s t i c s o f t h e p r o t o n s . T h e m o l e c u l a r e n v i r o n m e n t o f t h e p r o t o n s c h a n g e s , w h i c h i n t u r n a f f e c t s t h e i r NMR p r o p e r t i e s . I t i s t h e s e m o l e c u l a r c h a n g e s t h a t m u s t e v e n t u a l l y b e c h a r t e d , s i n c e t h e y a r e r e s p o n s i b l e f o r 73 the changes observed as gross pathology and on the NMR images. T h i s was the b a s i s f o r undertaking these s t u d i e s o f EAE i n primates. It i s known t h a t the f i r s t event i n EAE i s inflammation l e a d i n g t o haemorrhagic n e c r o s i s . E v e n t u a l l y demye1ination i s a l s o p r e s e n t . In t h i s p r o j e c t s e r i a l s t u d i e s have shown t h a t the p r o g r e s s i o n o f EAE can be f o l l o w e d u s i n g NMR imaging. Furthermore, measurement o f Tj and T^ as a f u n c t i o n o f time p r o v i d e s an i n d i c a t i o n o f change on a molecular l e v e l ±n v i vo. ( i i ) I nduction o f EAE and NMR Imaging P r o t o c o l EAE was induced i n a male Macaca f a s c i cu1ar i s monkey, weighing 3 kg, by i n j e c t i o n o f 0.15 mL o f a w a t e r - i n - o i l emulsion c o n t a i n i n g 15 mg monkey myelin b a s i c p r o t e i n and 0.5 mg heat k i l l e d Mycobacter i um tubercu1os i s i n t r a d e r m a l l y 54 in the h i n d f o o t pads . The animal was o b t a i n e d from the Bureau o f B i o l o g i e s Breeding Colony i n Ottawa and was housed f i r s t i n the UBC Animal Care F a c i l i t y on South Campus Road, then a f t e r i n d u c t i o n o f the d i s e a s e , he was moved t o the Acute Care Unit animal care f a c i l i t y . NMR imaging data were c o l l e c t e d on the P i c k e r I n t e r n a t i o n a l whole body imaging system u s i n g a r e c e i v e r c o i l with an a p e r t u r e o f 15 cm. Data were c o l l e c t e d u s i n g m u l t i - s l i c e spin-echo and i n v e r s i o n - r e c o v e r y p u l s e sequences, both o f which p r o v i d e d 8 contiguous 5 mm t h i c k 74 s l i c e s . E c h o d e l a y s ( 2 t ) o f 40 ms a n d 60 ms w e r e e m p l o y e d i n t h e s p i n - e c h o s e q u e n c e s . A i - t i m e o f 400 ms was u s e d f o r t h e i n v e r s i o n - r e c o v e r y s e q u e n c e , a n d i n a l l s e q u e n c e s t h e r e p e a t t i m e was 2 s e c . I t may b e t h a t t h e f i r s t l e s i o n o b s e r v e d i n t h e m o n k e y , w h i c h was n o t r e f l e c t e d c l i n i c a l l y , i s p a t h o l o g i c a l l y s i m i l a r t o c l i n i c a l l y a s y m p t o m a t i c MS l e s i o n s i n h u m a n s . C h a r a c t e r i z a t i o n o f t h e s e a s y m p t o m a t i c l e s i o n s i n MS 81 c o u l d p r o v i d e the i n f o r m a t i o n necessary t o understand and c h a r t the p r o g r e s s i o n o f the d i s e a s e . T h i s i s an e x c i t i n g t o p i c f o r f u r t h e r e x p l o r a t i o n . The progress o f EAE in primates can be e a s i l y f o l l o w e d u s i n g NMR imaging. The time o f appearance o f i n d i v i d u a l l e s i o n s c o u l d be noted, which a 1 lowed mapping o f the d i s e a s e post mortem. U n t i l now, t h i s i n f o r m a t i o n on the whole d i s e a s e process has been un o b t a i n a b l e . Accurate r e p o s i t i o n i n g i s e s s e n t i a l , s i n c e volume a v e r a g i n g c o u l d lead t o d i s c r e p a n c i e s i n the appearance o f the image, and in q u a n t i t a t i v e measurements, i f more than one t i s s u e type i s c o n t r i b u t i n g t o the s i g n a l observed from a p a r t i c u l a r s l i c e . Comparison o f the r e l a x a t i o n data o b t a i n e d immediately p r i o r t o death with the h i s t o p a t h o l o g y , r e v e a l s t h a t longer Tj and T 2 values a r e a s s o c i a t e d with the presence o f inflammation, haemorrhagic n e c r o s i s and d e m y e l i n a t i o n . In the i n i t i a l stages o f the d i s e a s e , not a l l t h r e e types o f pathology are pr e s e n t . The changes i n T j and T 2 over time r e f l e c t the molecular changes o c c u r r i n g due t o the p r o g r e s s i o n o f the d i s e a s e . The i n d i v i d u a l changes i n T j and T 2 from d e t e c t i o n u n t i l death (607. and 1407. r e s p e c t i v e l y ) a r e s i z e a b l e compared with the e r r o r s o f 127. and 157. found i n the s t u d i e s on water doped with paramagnetic s p e c i e s , d e s c r i b e d In Chapter 2. T h i s means t h a t the i n i t i a l l e s i o n can be d i s t i n g u i s h e d from those o c c u r r i n g l a t e r by these changes i n T. and T 9 . In oth e r 82 w o r d s , f t s h o u l d be p o s s i b l e t o d i s t i n g u i s h b e t w e e n a r e a s o f i n f l a m m a t i o n ( w h i c h i s a n e a r l y e v e n t ) a n d a r e a s c o n t a i n i n g d e m y e 1 i n a t i o n ( w h i c h o c c u r s l a t e r i n t h e p r o g r e s s o f t h e d i s e a s e . ) I n a d d i t i o n t o t h i s , t h e r e i s a n e l e v a t i o n i n T j b e f o r e t h e l e s i o n i s v i s i b l e o n t h e NMR i m a g e . I f t h i s e l e v a t i o n i s r e p r o d u c i b l e , a n d i s l a r g e i n c o m p a r i s o n t o t h e e r r o r s i n v o l v e d , t h e n t h i s i n d i c a t e s t h a t a f r e s h l e s i o n c a n be d e t e c t e d b y c h a n g e s i n T j b e f o r e i t i s v i s i b l e o n t h e i m a g e . T h e r e s u l t s o f t h i s w o r k h a v e s h o w n t h a t q u a n t i t a t i v e NMR i m a g i n g h a s t h e p o t e n t i a l f o r a n s w e r i n g many p e r t i n e n t q u e s t i o n s r e g a r d i n g EAE i n p r i m a t e s a n d e v e n t u a l l y MS I n h u m a n s . 8 3 CONCLUSIONS It has been shown t h a t r e p r o d u c i b l e v a l u e s o f the s p i n -l a t t i c e r e l a x a t i o n t ime ( T j ) , in the range 100-600 ms, can be o b t a i n e d u s i n g the i n v e r s i o n - r e c o v e r y method, a t a f i e l d s t r e n g t h o f 0.15 T e s l a on water doped w i t h v a r i o u s c o n c e n t r a t i o n s o f paramagnet ic s p e c i e s . I t has a l s o been shown t h a t chang ing the image s l i c e s e l e c t g r a d i e n t and the s p i n - e c h o read t ime do not a f f e c t the r e s u l t a n t T j v a l u e s . Us ing s c a t t e r in r e s u l t s o b s e r v e d under a range o f c o n d i t i o n s , i t has been e s t i m a t e d t h a t the u n c e r t a i n t y in the Tj v a l u e s o b t a i n e d u s i n g t h e i n v e r s i o n - r e c o v e r y method i s l e s s than 127.. T h i s method produces T j v a l u e s which a r e c o n s i s t e n t w i th the r e s u l t s o f t h e t w o - p o i n t computat iona l method, and in a d d i t i o n , makes e r r o r e s t i m a t e s p o s s i b l e . The i n v e r s i o n - r e c o v e r y method can a l s o p r o v i d e i n f o r m a t i o n on mu1t iexponent ia1 r e l a x a t i o n b e h a v i o u r , which i s Important f o r j_n v i vo s t u d i e s . The T j v a l u e s o b t a i n e d a r e w i t h i n 127. o f l i t e r a t u r e v a l u e s measured under c o n v e n t i o n a l s p e c t r o s c o p i c c o n d i t i o n s w i t h no magnet ic f i e l d g r a d i e n t s p r e s e n t . R e s u l t s show t h a t r e p r o d u c i b l e v a l u e s o f the s p i n - s p i n r e l a x a t i o n t ime ( T 2 ) in the range 40-200 ms, can be o b t a i n e d u s i n g the s p i n - e c h o method, a t a f i e l d s t r e n g t h o f 0.15 T e s l a on water doped w i t h paramagnet ic s p e c i e s . Above t h i s range the e f f e c t s o f d i f f u s i o n 84 become Important, r e s u l t i n g in abnormally low values o f T 2 , compared with l i t e r a t u r e values o b t a i n e d u s i n g the C a r r - P u r c e l 1 method, as m o d i f i e d by Meiboom and G i l l . The e r r o r s estimated f o r T 2 f n the range 40-200 ms are ± 157. and the r e s u l t s are w i t h i n 127. o f the l i t e r a t u r e v a l u e s . The spin-echo method p r o v i d e s T 2 values i n the range 40-200 ms, which are c o n s i s t e n t with values o b t a i n e d u s i n g the two-point computational method, and i n a d d i t i o n makes e r r o r estimates p o s s i b l e . The computational method takes the r a t i o o f two spin-echo images with d i f f e r e n t T,-values, which minimizes the e f f e c t s o f d i f f u s i o n and p r o v i d e s r e a l i s t i c v alues f o r T 2 longer than 200 ms. NMR imaging can d e t e c t Experimental A l l e r g i c E n c e p h a l o m y e l i t i s (EAE) i n primates b e f o r e the onset o f c l i n i c a l s i g n s . The technique can be used t o f o l l o w the development o f the d i s e a s e , which a l l o w s mapping o f i t s p a t h o l o g i c a l p r o g r e s s i o n . The p r o g r e s s i o n o f the d i s e a s e i s accompanied by a 607. i n c r e a s e i n T j , and a 1407. Increase 1 n T 2 .
