If the Beneficiary is Injured
(1) While in burning buildings. (2) While in
passenger elevators. (3) While riding as a passenger in or on a public conveyance provided by a
common carrier for passenger service, the Company pays
$5,000 for Accidental Death
5,000 for loss of both feet

For Dislocations and Fractures resulting from
injuries so received the Company pays one-half
the amounts mentioned under the head of
"Schedule of Injuries" in the policy.
For Surgeon's Fees necessitated by injuries so
received and referred to above, the Company
pays one-half of the amount payable to the
Assured for similar injury. (See Surgeon's
Fees List on this illustration.)
Insurance of Children Against AH Accidents
If any child of the Assured is injured, indemnity
depending upon the age of the child, and ranging
from $30 for one year to $520 for seventeen years,
is payable to the Assured should the injury result
i n death, loss of both hands, loss of both feet, or the
loss of one hand and one foot. For the loss of one
hand, or of one foot, one-half of above benefits are
payable.
The Company Grants Other Policies as Follows :
Fidelity Department—Against loss by defalcations of
persons in positions of trust.
Burglary   Department—For   bankers,   merchants,
householders and others against loss by burglary.
Plate Glass Department—Against loss by accidental
breakages.
Liability Department— 1.   Employers' L i a b i 11 y .
2.   Employers' Public Liability.   3.   Teams.
4.  Workingmen's Collective.
Boiler and Elevator Department—1.   Steam Boiler.
2. Passenger Elevators. 3. Owners' and Landlords' Liability.
Fly Wheel Department—Against, 1st loss of property; 2d,  damages to property of others; 3d,
damages for personal injuries caused by disruption or explosion of fly wheels.
Bonded List—Fidelity  insurance for persons employing lawyers to make collections.

$5,000 for Accidental Death $5,000 for loss of one hand and one foot
5,000 for loss of both hands 5,000 for loss of one hand and one eye

5,000 for loss of both eyes 2,500 for loss of one hand
2,500 for loss of one foot 2,500 for loss of one eye
And Pays in Addition
to the foregoing a further indemnity of $25.00 for each week of total disability between the
date of the accident and the date of death, dismemberment, or loss of sight.
Where specific indemnities for specific disabilities are not provided, the Assured is entitled to

For Accidents
$25.00
for each week of Total Disability from date
of accident, but not to exceed 200 weeks;
also
$22.50 to $6.25   *
for each week of Partial Disability within
200 weeks from date of accident, but not to
exceed 26 weeks in duration. The amount
per week depends upon the extent of the
disability;
Also Indemnity at the Rate
of $325.00 a Year
for each week of Total Disability beyond
the 200th week of disability and terminating
at death or when Assured reaches the age
of 70.
Double Indemnities
Each of the foregoing indemnities-is
doubled if the injury is sustained by the
Assured (1) While in burning buildings. (2)
While in passenger elevators. (3) While
riding as a passenger in or on a public
conveyance provided by a common carrier
for passenger service. (4) While in falling
buildings.    (5) If struck by lightning.
For Illness
$25.00
for each week of confinement to the house
necessitated by illness of the Assured from
any of the 50 diseases listed above, not to
exceed one year, and also
$12.50
for each week of subsequent Total Disability
during convalescence, not necessarily confining Assured to the house, n6*t to exceed,
however, one year from date of commencement of illness;
Also Indemnity at the Rate
of $325.00 a Year
for each week of Total Disability beyond
the first year after commencement of illness
and terminating at death or when Assured
reaches the age of 70.
For Quarantine
$25.00
a week, not to exceed 10 weeks, that Assured is involuntarily quarantined.
Hospital Indemnity
In addition to the regular Weekly Indemnity but not exceeding
$12.50 a Week
for Any Illness Listed Above or Any Accident
is provided if the Assured is confined to a regularly incorporated hospital.    Limit 10 weeks.
Double Indemnities are payable if the
injury is sustained by the Assured
(1) While   in   burning   buildings.    (2)
While in passenger elevators.    (3) While
riding as a passenger in or on a public
conveyance provided by a common carrier for passenger service.    (4) While in
falling buildings.    (5) If struck by lightning, as follows:

An Identification Card enclosed in a
neat leather card case is furnished by the
Company with this policy.
Sunstroke, Freezing, Asphyxiation,
Hydrophobia, and Blood Poisoning
are construed as bodily injuries.
Surgeon's Fees as shown by this circular are payable in addition to weekly,
optional, or fixed indemnities; excepting
where hospital indemnity is paid.
For Accidents which do not disable
the Assured, but require  minor surgical
aid,   Surgeon's   Fees to  the  amount -..qfef
$25.00 are payable.
From $3,850 to $6,250 are payable
when accidental injury results in
Insanity.
From $2,550 to $6,250 are payable
when accidental injury results in permanent Paralysis.
$2,500 for Total  Blindness  or Total
Paralysis from disease.
Every feature illustrated above, as well as those on the back  relating to Beneficiary and
Children of Assured, is contained in the Policy.
