With reference to your report on Form li+09 of an
incident on board the "DORIS YORKErf at Vancouver
on March 13, 1972 when Doman Tractor No* 211+ was
damaged as a result of being struck by the shipTs

during loading of the "DORIS YOREE" at Vancouver* B.C#
on March 13, 1972♦
If It is anticipated that cost of repairs will exceed
the ^50*00 deductible under the "Cargo, Gars and
Contents" insurance, I assume that you will arrange
for a survey of the damage by the Salvage Association*
London and in due course let me have the usual documents
for claim on insurance*

Second Mate on the DORIS YORKE, concerning damage
to Doman Tractor 214 on March 13th.
This matter is presently being developed between
Doman1s Transport Limited and F#M. Yorke & Son
Limited.

of the report from W. Burnett, second mate on the
"Doris Yorke", concerning damage to Doman tractor
214 on March 13th.

At approximately 22:40 on March 13, 1972 Doman Tractor
while, coupled to a trailer unit, backed on to park his
trailer on the main deck of the "Doris Yorke".  After the
backing up units passed clear of the raised elevator I
pressed the button activating the two separately located
automatic flashing warning lights and signs, indicating
the elevator is in operation, as well as the four flashing
corner clearance lights located on the elevator platform.
Eight seconds after this the elevator automatically
proceeded downward.  As the elevator was coming down I heard
the tractor coming forward.  I immediately pushed the stop
button stopping the elevator about S1 from the deck.  At
the same time I flashed my flashlight and shouted at the
driver to stop.  The driver did not stop until after he came
in contact with the rear of the stopped elevator.  The
damage occurred about 2 0 seconds after the warning lights
had been  put into continuous operation.
The reason the tractor came forward was apparently
because he wanted to manoeuver his trailer unit into a
different parking position than he had originally backed
into.  If the driver had been watching the various warning
lights and signs he would not have driven forward into the
elevator.  My impression was that the driver never did look
ahead but was keeping his eyes on his rear view mirrors or
looking back completely.
Damage to the tractor consisted of a pushed in cab and
broken windows across the front at eye level, and doors
sprung and top of cab at center pushed up all resulting
from the tractor being driven in to the end of the elevator
platform. :fh
