卷四未完，似有殘缺。卷五分為兩部分，頁碼分別起訖。
無欄框，書27.5 x 16公分，9行18字，白口，無魚尾，版心上題書名，中題卷數，下題葉數。
首萬曆丁未春正月鳴山外史黃淳“重修崖山新志敘”，及許炯、張詡崖山舊志序。目錄題名“重修崖山志”。
徐信符印：“南州/書樓”朱文方印。
姚鈞石印：“姚鈞石/藏書”朱文長方印；“蒲坂書樓”白文長方印；“民國/庚辰”朱文方印。
