Feature Article: Consultation Notes in Pharmacy: More than just Pen to Paper. Consultation notes are a style of documentation used by many health care professionals including the Clinic team. Recently we began investigating strategies to improve and optimize the way we write consultation notes and are sharing commonly encountered documentation traps and suggestions for writers. Case: Leveraging lifestyle for low blood sugars. Pharmacists are accustomed to counselling patients about hypoglycemia management; however, this issue’s case introduces a patient with post prandial hypoglycemia (PPH) and explores different treatment strategies including medications and lifestyle.
