Aesthetic Outcomes of Patient with Unilateral Cleft Lip and Palate Following NasoAlveolar Molding (NAM) Therapy in an Outreach Setting, in Guayaquil, Ecuador Kassam, Serena
Abstract
Purpose: To Assess a model integrating long term delivery of pre-surgical NasoAlveolar Molding (NAM) therapy as a part of sustainable comprehensive cleft care treatment, in an outreach setting. Methods: Using the Cleft Aesthetic Rating Scale (CARS) and a modified scar rating scale, to assess aesthetic outcomes of patients who had received NAM were compared to their non-NAM controls. Intra and Inter- rater agreements were also evaluated. Results: Using the CARS and a modified scar rating scale, patients who had received NAM were found to have superior nose, lip, and scar aesthetic outcomes in comparison to our non-NAM controls; CARS Nose, CARS Lip, Scar Color, Scar Heigh, Scar Width Overall, we saw a trend toward better outcomes for patients treated with NAM vs no-NAM. Overall intra- rater agreement across all assessment points was strong and moderate for inter-rater agreement. Conclusion: Our study has shown that NAM patients trended toward improved scores for Nasolabial esthetics in Unilateral CLCP and Scar outcomes for both Bilateral and Unilateral CLCP patients treated in Guayaquil Ecuador with GSF. However more investigation is needed to show universal statistical significance in our results. Our results suggests that NAM is not only feasible in an outreach setting, but it is also shows promise with improved lip, nose and scar outcomes.
