Abstract
Aims: Nurse case managers (NCMs) are essential in supporting people living with heart failure (HF) in the community. This Scholarly Practice Advancement Research Project (SPAR) provides evidence-based knowledge to optimize the role of nurse case management (NCM) in reducing HF readmissions in the home health setting. I conducted a review of the literature on the impact of NCM on HF readmissions to determine areas of practice and effective strategies for managing HF. My goal is to help close the theory-practice gap in case management practice and advance its role in reducing HF readmissions and caring for people living with HF in the community.
