Therapeutics Letter 88 explores the use of pragmatic randomized trials as a means of addressing gaps in evidence. Compared to efficacy trials, pragmatic (effectiveness) trials are intended to answer the questions that clinicians and patients have regarding therapy. Gaps in evidence may related to the need for more representative subjects in trials, the need for better assessment of harms, as well as the need for more meaningful patient-oriented outcomes.
